- any authorization information, especially if your condition is work or auto related
- name of referring Physician
This data will allow us to verify your insurance’s terms of coverage for Physical Therapy services, so you could make an informed decision ahead of scheduling actual appointment. Once Appointment is scheduled we will call you an evening before to confirm it.
In case you need to reschedule or cancel initial or any future appointments, please, call us in advance, possibly 24 hours.
