what is "referral"?

  • Referral is also known as a “script" or "prescription“ given to you by Physician and should be used within 30 days.
  • Medicare clients’ prescription is valid for 90 days.
  • Expired prescriptions need to be renewed. In case if client does not have a Family Physician we can recommend a Physician for them to see.

Who may refer you for Physical Therapy?

Doctors:

  • Medical Doctor (MD)
  • Doctor of Osteopathy (DO) 
  • Podiatrist (DPM)


Specialists such as:

  • Orthopedic Surgeon
  • ENT
  • OBGYN 
  • Urologist
  • Ophthalmologist
  • Neurologist
  • Pediatrician 

On behalf of Physician:

  • Physician Assistant (PA)
  • Nurse Practitioner (NP)
