Please, arrive 15-20 minutes early, so we could review insurance benefits information and complete intake forms.  You will need to bring with you:  

• Physical Therapy prescription
• Insurance Card 
• Driver’s License 
• List of current prescription medication  

You may bring completed New Patient Forms: Initial Evaluation Intake form, HIPPA Consent (privacy policy) and the Questionnaire which relates best to problem you will be seen for. Forms are available under "Forms" tab to download from our website.

Physical Therapist will perform Initial Evaluation consisting of: 

• History of your condition  
• Tests and measurements as applicable for future objective assessment of changes 
• Establishment of functional Goals reflecting your expectations  
• Initial Treatment of your condition.  

Scheduling of further appointments: it is recommended to schedule all sessions in order to secure days and hours that will best fit your availability.
